Health Forms Required

1. Camper Medical Information Form
This online form provides essential information for our healthcare staff in the event that your child needs care while at camp.

2. Proof of Physical
We require documentation of a physical exam within one year of your camper’s session start date. You may:

• Upload your doctor's physical paperwork, OR

• Download and print our Health History & Examination Form for your doctor to complete.

Upload completed forms through your Campwise account or email them to jcicconi@ligoniercamp.org.

Please ensure all forms are submitted before your camper’s session begins.

Whitewater Rafting Information

Campers attending Next Level will go whitewater rafting at White Water Adventurers in Ohiopyle, PA.
Rafting is also available during Classic Camp 4 for campers ages 12+ for an additional $85.

All participants must have a waiver completed by their parent or guardian.
