Health Forms Required

1. Camper Medical Information Form
This online form provides essential information for our staff in the event that your child needs medical care while at camp.

2. Proof of Physical
We require documentation of a physical exam within one year of your camper’s session start date. You may:

Upload completed forms to your Campwise account or email them to jcicconi@ligoniercamp.org.

To upload to Campwise, from the Dashboard of your account, click Forms (Manage Camper Forms). Under Download/Upload Forms, select Upload Forms. Choose either Health History and Examination or Immunizations (Optional), then click Choose File to select the completed form from your device. Once the file is selected, click Upload to submit the form.

Please ensure all forms are submitted prior to arrival.

Whitewater Rafting Information

Campers attending Next Level will go whitewater rafting at White Water Adventurers in Ohiopyle, PA.

Rafting is also available during Classic Camp 4 for campers ages 12+ for an additional $90.

All participants must have a waiver completed by their parent or guardian prior to arrival.

When completing the waiver, use the Trip Date and Booking ID below and select "Lower Yough Guided."
